Office Policies
OFFICE POLICIES As our primary focus is to ensure patient satisfaction, we have developed the following policies regarding our office. Insurance: We participate with many insurance plans (see a staff member for the most recent list). If we are a participating provider for your plan, we will be happy to bill the company for you. You will be responsible for any co-pays and/or deductibles at the time of your visit. When we receive a statement from your carrier, adjustments may be necessary and you may receive a statement from us. Regardless of coverage status, you are ultimately responsible for any unpaid insurance claims. Keep in mind that some companies only cover routine eye care and some only cover medical eye care. Most insurance companies do not cover a refraction, which is when the doctor determines your prescription for glasses. You will be required to pay for the refraction, currently $40.00, at the time of your visit. If your insurance pays for this service, we will reimburse you with either a credit toward any other balance or with a check. Although we try to know your policy prior to your exam, every company has different policies and it is difficult for our office to know every detail of every person’s plan. Payment for Services: All payments are due at the time of service unless other arrangements have been made in advance of your visit. We accept cash, checks and most credit cards. Ask a staff member for approximate pricing based on the reason for your visit. If we are not a participating provider for your insurance plan you must pay us directly at the time of your visit. We will be happy to provide you with any paperwork necessary for you to submit for reimbursement. Returned checks will be subject to a $25.00 processing fee. Refractions and Contact Lens Fittings: Please read additional handout regarding refractions and contact lens fittings. If you did not receive one, please see a staff member. Appointments: We try our best to accommodate our patients and give them appropriate time with the doctor. Since we are reserving time especially for you, we ask that you kindly give us 24 hours notice if you are unable to make your assigned time. We would be happy to reschedule you to a more convenient time. Although we understand that emergencies do exist, we reserve the right to charge you a $30.00 cancellation fee if you do not notify us within 24 hours of your scheduled appointment. Optical Department Policies: Since eyeglasses and contact lenses are a special order item, we request that you pay in full at the time of your order. No orders will be processed until at least 50% of the total bill has been paid. We spend a great deal of time taking measurements to ensure the proper fit and type of lenses appropriate for your needs. Cancelled orders will be subject to a $10.00 service charge. Frame Warranty: All frames purchased from Advanced Eye & Laser Center come with a one-year defective product warranty. We will either repair or replace your frame with the same or an equivalent value frame at no charge. You may be responsible for shipping and handling charges. Eyeglass Lenses: We work very hard to recommend the most appropriate lenses for each individual. We use the highest quality optical lenses from the best labs in the country. We will verify your prescription and make sure you are happy with your purchase. Since we special order and cut every pair of lenses sold, we cannot offer any refunds on lenses. Ask our Optician for any specific warranties on the lenses you choose. You always have the option to purchase a scratch warranty for extra protection. Progressive Lenses: We understand that some patients require time to adapt to progressive eyeglass lenses. If you are having difficulty with the lenses we ask that you speak to our Optician and explain your issues. Adjustments may be necessary. If within 90 days you are not comfortable wearing the lenses, we will be happy to replace the lenses at no charge to either lined bifocal or single vision lenses. If possible, we will keep the same or similar options on the lenses (i.e. anti-reflective coating, tint, photochromatic, etc.). Since these are special order lenses we are unable to offer any refunds. Return Policy: All sales are final. We will give you 100% credit for your frame toward the purchase of another frame or lenses if returned within 10 days of receipt. Frames must be in perfect resale condition. We will give you a 50% credit for your lenses toward new lenses or a frame if returned within 10 days of receipt. Unopened contact lens boxes may be exchanged for full credit. Finance and Collection Charges: Since we are a relatively small business we rely on prompt payments from our patients. We will charge the maximum interest rate allowed by law for any balances left on your account. This includes any unpaid insurance balances. Additionally, if we need to use a collection agency to obtain payments from you, you will be charged any collection costs imposed on us by the agency. Collection fees and finance charges could greatly increase your original balance so we urge you to pay promptly. Overpayments: If we are reimbursed by your insurance company for fees that you already paid, we will promptly credit your account. If there are any unpaid balances on your account we will apply the credit to these charges first. Any credits remaining on your account may be applied to future charges or given to you as a check. Please advise the staff of your wishes if you are expecting any insurance reimbursements. Any other overpayments that we receive shall be applied in the same manner. Again, please remember that our primary focus is to ensure patient satisfaction. We strive to give our patients quality healthcare at affordable prices using the finest and safest technology available. We also believe in allowing patients considerable one on one time with the doctor. This is unusual in a time when patients are likely to spend five minutes or less with their doctor.
